Delve into the world of neonatal and infant PICCs with our exciting and up-to-date course. Designed in alignment with the recommendations of the National Association of Neonatal Nurses and the Infusion Nurses Society, this course provides comprehensive insights into PICC placement for neonates and pediatric patients.
We kick off the course by exploring vascular access options specifically tailored for neonates. You will gain a solid understanding of vascular access devices and the significance of PICCs in this population. We leave no stone unturned as we dive into various aspects of PICCs, including:
- Indications and contraindications for use
- Growth and development considerations and their impact on device selection
- Neonatal vein anatomy and vessel health
- Step-by-step insertion procedures tailored to neonates
- Complications that may arise and how to address them
- Proper care and maintenance to ensure optimal outcomes
- Legal aspects specific to neonates
Program Objectives and Goals
- Define peripherally inserted central catheter by acknowledging the Infusion Nursing Standards of Practice designation for terminal tip location for neonates
- List the advantages and disadvantages of PICC placement
- State the indications and contraindications for PICC placement
- Review major anatomical features of veins
- Describe the main and alternate veins utilized for insertion of catheters with neonates and infants
- State considerations for patient selection and parental involvement
- Describe the proper measurement procedures for all possible insertion sites
- Describe ways to reduce trauma in neonates requiring vascular access
- List the steps of the PICC insertions procedure as related to neonates
- Describe the procedures for managing catheter malposition
- Explain neonatal care and maintenance procedures acceptable for use with PICCs including flushing and dressing changes
- Recognize potential complications associated with the use of PICCs
About the Additional Component of the Neonatal PICC Training:
Simulation and supervised insertions are vital components of training for safe neonatal PICC insertions. Attend hands-on workshops or work with your supervisor to practice simulations and observe actual patient insertions with ultrasound guidance. While there is no set number of supervised insertions required, it is crucial to perform enough to feel competent and confident in handling various situations. PICC insertion is an advanced practice that should only be performed by specialists with proper training and demonstrated experience. Supervisors ensure sterile technique, manage complications, facilitate learning, and contribute to successful insertions. Education, competency, and documentation are essential for reducing medical liability. Utilize provided competency forms and the Global Rating Scale to verify your proficiency under supervision.
The Steps:
1. Successfully complete this course and the course post test
2. Print your continuing education certificate to use as proof of successful completion of a Neonatal PICC Training program.
3. Begin your supervised, simulated insertion
4. Followed by supervised insertions on patients to the point of competency, comfort and confidence
Note: PICC training is a process and includes not just one course, but many courses. Continue to seek out educational opportunities including but not limited to ultrasound training, complication management, and tips and tricks in a variety of areas.
Ultrasound Requirements:
Ultrasound is strongly recommended as a safety component of PICC insertion. In the past ultrasound insertion for pediatric and neonatal PICC placement has not been well accepted. The trend now is to incorporate this technology to add a level of competence that improves success, allows for expanded vein selection and provides more options for improved outcomes with neonates. Ultrasound cannot be learned in one or two lessons. In this course we introduce ultrasound as one of the options for insertion. In the beginning ultrasound can best be used for vein assessment and selection. As you gain in mastery add ultrasound guided needle insertion, especially when veins are deeper. Use of phantom vein pads to teach hand eye coordination is helpful in gaining skills with ultrasound guided needle placement. Using ultrasound for not only PICC placement, but also for ultrasound guided peripheral catheter placement will provide more opportunities for practice and lead to fewer overall attempts at device placement. Additional ultrasound courses are available to aid in understanding, provide tips and tricks while helping to facilitate your process through the learning curve with ultrasound. Simulation work and one on one coaching during insertions with a clinician experienced with ultrasound will greatly aid your understanding.
Another Consideration:
As you continue to perform insertions you will reach a level of qualification to become certified. After 25 insertions and documentation of supervised competency you are eligible to apply for the CNPI Certified Neonatal PICC Inserter program. Following approval of qualification, a study guide is provided and sample test to assist you prior to completing the online proctored examination. Successful completion of the CNPI examination allows you to receive this professional designation, effective for a three-year period. An easy 3 step renewal process ensures you will retain your certification. Receiving this designation and maintaining your CNPI status places you in an elite group of expert clinicians who have demonstrated qualification, competence and knowledge necessary to provide the highest level of patient insertion and care with PICCs.
